Post-Baccalaureate Certificate • HHS C MGT PB

The Health and Human Service Case Management Certificate program is designed for working professionals who have obtained a bachelor’s degree in a science, health, or human services related profession and want to pursue or advance their career as a case manager. The student will learn the components of health and human service case management including assessing, planning, implementing, coordinating, monitoring and evaluating the services of a client to improve their quality of life. The student may be eligible to take the Certified Case Managers (CCM) Exam and the Human Services Board Certified Practitioner (HS-BCP) Exam.

Preadmission Requirements:

Successful completion of an undergraduate degree earned at an accredited college or university as well as successful completion of Anatomy & Physiology equivalent to BIOL 115 and BIOL 116 . Student will need to complete BIOL 116 or BIOL 222 /BIOL 222L prior to beginning the Health and Human Service Case Management classes.

Core Courses - 20-22 Credits

HHCM 300 - Case Management Fundamentals 3 Credits
HHCM 312 - Insurance, UR, and Discharge Planning 3 Credits
HHCM 401 - Documentation and Reporting 4 Credits
HHCM 402 - Disability Management 3 Credits

Choose 7-9 Credits from the following:

HHCM 310 - Community Services 3 Credits
HHCM 311 - Adjustment to Disability 3 Credits
HHCM 403 - Special Populations 3 Credits
HHCM 495 - HHS Case Management Practicum 4 Credits *

Total Credits: 20-22

**HHCM 495 - HHS Case Management Practicum is required if students have not previously completed a supervised field experience in case management, health, or behavioral health in their baccalaureate degree program.
